In terms of the Capability Maturity Model (CMM), the vast majority of software companies are at level 0 to 1. Developers in such environments are totally incapable of "doing it right the first time". Tech writers (downstream of design) can not hope to do better (unless one has a very loose interpretation of what "doing it right" means).
