Thank you to everyone who replied.
Most of you suggested something similar to this from Roger Bell:

Unfortunately, it didn't <wry smile>.
As far as I can tell, the correct items are checked.
When I create the PDF, the Bookmark pane displays properly.
There are no bookmarks in the Bookmark pane.

I created a new Word doc, and moved the sections over chunk by chunk,
verifying each heading style. The TOC in the Word doc (3 heading levels)
is fine. All the links work. I created the PDF, making sure that the
options were set to create bookmarks from the same 3 heading levels.
Neither Distiller or PDFWriter gave me links in the Bookmark pane.

I give up.
I've told the client that, if she wants PDF bookmarks, I
will have to create them manually and she will be charged
for the time. I'm waiting to hear from her now.
