Actually, I worked with long documents in Word. I split em when they were about 3 Mb and choose to paginate on a A-1 basis, 'cuz IMO, it makes index more efficient (you know which part of the book is referenced).
You're right, NEVER use master documents, it doesn't work
You can indicate manually the page number where it should begin, that' a little chore, but AFAIK, word is the most frequently used WP in Europe.
