It's worth maintaining on-line and print versions of your portfolio, but
I'd keep each complete in itself for readers' convenience. Maybe you
could take a few screen shots for your paper portfolio?

Why not? If it's technical stuff, it will show your ability to handle
complex subject matter in a variety of forms. And, even if it's not
technical, it could show your ability to do design work.

Why not scan a few pages? It's easier, and takes less time.
But if you do re-write, clearly label it as such. Maybe you could
present a before and after version of the manual.
I'm assuming, of course, that you have permission from the bank to use
the chapter.

Again, screen shots might be the way to go. You could fit a couple per
page and still have the help file legible.
