Annamaria,
I did all these things. I just can't get the chapter heading (which is in my
header) to appear on the TOC. When I toggle the field codes, it shows up.

Kim,
Under Index&Tables, when you choose TOC, before you close out, you also
need to choose "options" and set the levels manually. I haven't figured
out how to set custom options as the default yet. But that's where you do
it. But before you do that, you have to set the chapter title as Heading 1
or 2, etc. If there's an easier way, I haven't found it yet! ; ))
