We've updated the TECHWR-L Poll with a new question:

In your current job, when doc problems are found, what
happens (primarily)?

* Fingers point and blame is distributed
* Fingers point and blame is unfairly distributed
* Someone takes responsibility
* Someone takes responsibility, even if not at fault
* Analysis of cause or problems undertaken
* Procedures and processes change to prevent repeats
