Quite a few places I've worked have held a post-mortem after a project
ends. This basically meant that the project was looked at in detail to
see what worked, what didn't etc with the idea being to improve things
for the next project. TWing was an integral part of this process and
covered things like - how was the information flow, timeliness of
reviews etc. It was fairly informal but there was a report generated for
larger projects.
