The easiest way is NOT to try to screen capture it.
Instead, in word,
1.Insert ->Object
2. In the "Create from file" tab, select Link to file, browse to the form
and click OK.

The form now appears inserted into your word doc. You can resize the form as
for any other graphic, without losing legibility.

Works for me, anyhow
