Web Survey Software: Recommendations and Suggestions

Hi,
A colleague of mine needs to conduct regular Web surveys and is looking
for suggestions on software packages. Her requirements are pretty
standard, but I don't know much about what's out there. I've only used a
couple of kludgy chunks of PHP code which aren't ideal for her needs.

There are plenty of products out there. Here are a few I've found:
