I don't know whether this is the right information you want because your
mail says that the purpose of the document is to troubleshoot servers but
you have named the document as a server test plan. If you are looking for
a template for documenting possible errors/problems and troubleshooting
instructions, use a format like this:

o Server specifications
-Backup policy
-Devices connected to the server

o Network specifications
-Details of workstations and peripherals
-Approximate utilization of resources
-Resource allocation details
-Shutdown and restart procedure in case of emergency

o Details of support contacts
o Problem ID/description
-Troubleshooting procedure

o Common error messages and description
