How about "Post Project Assessment".

Basically you want to find out what went well, so you can reproduce it, and
what did not go so well, so you can avoid doing those things again. So you
ask, "What went well? What could be improved?"

One way to collect all this input is to have people write their responses (5
for each question) on sticky notes and stick them on a wall. Then, as a
group, you move them around into categories. Then, you write up the
results, file them away, and continue on with business as usual (kidding).

There are various ways to get input. You can so a web search on
brainstorming techniques or strategic planning techniques and find some
other ways for gathering and classifying input from a group.

The important thing is that you say you have some enthusiasm for this. That
is good. Don't even bother to do it if the desire and the ability to
actually use the results to create change is not there.
