This is the sole purpose of HTML Indexer.

The free demo version will show you exactly how to use it to index your HTML Help project.

I don't think there's a better tool for indexing HTML files, including HTML Help and JavaHelp...but my opinion may be affected somewhat by my close relationship with the developer and my deep interest in the product's continued success.
