I have seen it happen in those circumstances and also when I open a doc, print it to a printer OTHER THAN the one I printed it to last time, and then close the doc.

Apparently it wants to save the print set-up

A couple of instances I have seen this happen is when (a) I run spell check (even though no spelling changes were made) and (b) I go into "view footnote" mode, but don't make any changes.
