A story on CNN.com, http://www.cnn.com/2004/US/Southwest/07/28/drowning.ap/index.html, described the results into the investigation of the drownings of 4 persons at a Texas water park. I find interesting that it was noted that: "it never had an operating manual" and the knowledgeable workers who trained newcomers, were let go a couple years ago to cut costs. Of couse, there are other contributing factors, but lack of documentation and training were the major factors.
