As Bruce said, it is quite common but some companies/managers DO care
and make the effort to learn and some learn the hard way. At one company
we did a project for the military and the project manager didn't factor
docs into the process at all - tech pubs'll 'knock something together'.
That 'something', written to exacting military specs, cost more than the
profit margin for the whole project. We were always asked after that!
