I use DBDesigner for documenting our MySQL schemas:
http://www.fabforce.net/dbdesigner4/

It claims to work for Oracle too, but I haven't tried it. It's FOSS so
you have nothing to lose. Use the 'reverse engineering' feature to read
in the schema, then use the included HTML plugin to make a nice HTML
listing by table. You can add comments to document each table and
field.
