I also find using a trackball much harder on the hand
than a mouse. For me, it's because my wrist is
stationary while my hand and fingers roll the ball. I
have the same problem with the scroll wheel on the
mouse.

I tried a mouse pad with a wrist gel support, but it
made things worse. Again, the wrist rests and is
stationary, so the hand and fingers do all the moving.
For me, moving the mouse with the whole arm works
best.
