Sarah,

Boy, do I feel for you! It's a rotten feeling when something incorrect
goes through in your documentation, whether you could have done anything
about it or not.

Rather than getting mad (easy to say but not to do), just shrug and tell
them, "I'm sorry. That's the information I was given. No one pointed out
to me during the review cycle that it had changed, and I had no way to
check it myself. If you want this changed, here's what we need to do.
And if you want this not to happen again, here's what needs to happen."
That's about all you can do at this point.

BTW, what the head of tech support said has to be the lamest thing I've
ever heard anyone say for letting a mistake get by him. If you sign off
on something, that means it's correct to your knowledge. If you didn't
review it, either don't sign the review sheet or be prepared to accept
the blame if something's wrong. I agree with Gene. Show up at his desk
prepared to stay for a long time--even if he's on the phone. Spread out
your work or your lunch and make yourself comfortable.
