Before you try Geoff's solutions, try selecting the whole doc and updating fields (F9). If the fields are based on styles and not bookmarks, that might fix it for you. If they are based on styles (and they're still messed up), you'll have to figure out what styles they were using for what purpose (Geoff's instructions work for that, I think), and then fix the apply the styles appropriately.
