Never.  At most, if I'm going out because of a reduction and
there isn't already one in the separation package, I ask for
a statement that my termination was due to a general
reduction in force.

My preferred source of references is managers I've worked
closely and well with at a company who decide to board the
train out the door before I do.  Anytime someone I've had
a good working relationship with leaves a company I'm at,
I ask them if they're willing to be a reference once they're
out, get personal contact info, then keep it up to date forever.

Interesting... Although I agree with Collin that the letter of
resignation may not be the place to ask for a recommendation, I am
interested in having a my co-workers recommend me.  Since I already have
a position lined-up, is a recommendation really necessary?  I fully
intend to not 'burn-bridges' because I really do like the people I've
work with and the current company... My leaving is more of a career
choice.

When do you ask for a recommendation, and when do you let sleeping dogs
