Thanks to those who responded. I found two good free PDF converters:
CutePDF and PDFCreator (not to be confused with "PDF Creator" or
"PDF-Creator"). Neither, however, enables one to create interactive
PDFs.

I noticed that you can't save the Open Office form, Richard.

I haven't used Nitro PDF yet, but they have had a special, $49, and
their claims imply they match Acrobat Pro capabilities.
