To me those are just deliverables to be generated from a common
source. I very rarely think about them while writing. It doesn't
matter whether I'm delivering online help only, PDF only, Web only, or
two or three of those.

I used to write reviews for CNET, and it wasn't that different. The
outline had to be conceived in such a way that it could be delivered
as a set of pages, which is pretty much like the topic-oriented
approach that's standard for technical documentation.
