I'd call it a checklist. At the top would be a headline instruction, "Before you start..."

Next there would be one or two sentences explaining what to do and why. The items in the list could be a combination of checkboxes and blanks to fill in. Some of them could indicate that they'll need to be deferred until further information arrives. A partially filled-in list could then become an action item for some other person.

I am contemplating a fillable form/cheat sheet/check list. Some items in
the "list" are pure check list items, such as "check that your VM
