First, Word 6 will do exactly as you describe. Look in the topic
"Marking Index Entries" in the Word Help for details. (Basically, just
highlight the desired text and press ALT+SHIFT+X.)

But it also sounds as if you want to create a concordance rather than
a true index. For indexing, you can just place the insertion point
where you want the index reference, press ALT+SHIFT+X, then type the
text of the index entry.
