Bob Morrisette's response to Caryn Rizell's comment on a
manual index being better than an online index:

============== I'm not sure why a manual index is superior
to an online index. They should be the same. If you use
FrameMaker, you can generate an index with hypertext links.
What could be quicker than that?

Bob, you've answered your own question. If the online index
is created with hypertext links, it's missing a lot.
Concepts that are not spelled out explicitly in the text are
not indexed. A manual index is likely created by a human
indexer, and would pick up those missing concepts. That's
why the manual index is far superior!

And quicker is not necessarily better!
