Sorry, no studies to quote either way on this, but sentence case is
probably the best. I have heard people complain bitterly that all_caps
feels like you're being yelled at. Also, if everything is in caps,
everything is screaming for your attention - leaving your headings,
etc., lost in the noise. Perhaps in handwriting, all caps is a good
thing (because few people have legible script), but mechanically
produced pages (forms or not) usually look better in sentence case.
IMHO....
