The conventions in our docs are:

name of screen/dialog box in italics (the _Open_ screen)

name of buttons/keys in bold small caps (press *ENTER*)

text to be typed exactly in a monospaced font (I can't begin to try this in
ascii)

information to be typed in bold italic small caps (enter your _*LOCATION
IDENTIFICATION NUMBER*_)

I'm lobbying against that last part, which I think looks awful, but so far
I'm being outvoted by the rest of the team.
