A recent problem in numbering was solved this way:

I had more than one Chapter/Appendix para tag example in each template
chapter, therefore it was skipping numbers/letters when autonumbering
these in a book.

The solution: change the para tags to body.

(Ouch! Pick one: humiliation or humility. There is no third choice!)
