The trick to this is RD (Referenced Document) fields. You can find the details in the help under "Field Codes" for some obscure reason. The overall TOC doesn't update "automatically" but all you have to do is run the TOC when you're ready and voila, it's updated.

If you need more help than Word Help provides (and no one will be surprised if you do) let me know.
