You might want to check out the article "X Marks the Spot" by Michael Skeet,
which I've published on my web site at

Mike describes a system that he was using at OpenCola, which allowed him to
produce HTML and printed docs from XML source. I did see this in action and
it was pretty neat, but it took a lot of work to set up (not to mention the
services of at least one real XML guru). They're now using FrameMaker and
WebWorks Publisher, incidentally -- it's much easier for mere mortals.
