Depends on what word "only" really modifies... "included" (as an adverb) or
"factors that . . ." (as an adjective).

"Factors that are the same across all pipelines and internal companies are only
included in this form-- not included in the other form, and not broadcast on the
evening news."

vs.

"Only factors that are the same across all pipelines and internal companies are
included in this form-- no other factors are included."

Your instinct that her version sounds better is right on target. In my
experience, that kind of common-sense hunch is often more reliable than going
through reference books. (This reminds me of the time I was a little kid
experimenting with nail polish, and I stubbornly insisted to my mother that
"Apply to clean nails" meant the stuff cleaned the nails, since "clean" was a
verb... In that case, a little grammatical knowledge was a dangerous thing.)
