Format > Page Layout > Apply Master Pages, or checking Apply Master Pages when updating a book will create the page and place a blank table. Or, you can add the page MasterPageMaps--note no spaces-- and build the table from scratch. Once you have the table, you populate it with the paragraph format and master pages to apply. The first couple times can be tricky, but I've found it's one of the best "tricks" FM has learned. Once I figured out how to do it, I've been using the function regularly.

BTW, once you have it set up, and if no pages have changed, you do not need to select AMP everytime you update.
