The best you can do is to keep it as simple and small as possible.
Everyone will want to have their finger in the pie.

For the group that considers change requests, try to keep it out of the
upper management, better at a lower level, and if possible delgated to
lower management levels, or senior SMEs. As few people as possible in
order to make it managable is best.

The form should be along the lines of problem, solution. Benefits to the
company probably are also a good thing for an enhancement form to include
as well.

It would probably work better to keep the triage group to four or less
people. Just include the departments that are vitally concerned,
sales/marketing, engineering (R&D), manufacturing, whichever departments
are concerned. All others can be consulted after the initial triage, or as
research prior to triage.

Should require no more than three signatures to get an enhancement moving.
It should then go to the managment group for consideration as a project or
inclusion in a project.

At this stage of a companies life, it should be kept as simple as
possible, or the process will paralyze you, or get dumped because it isn't
workable.
